Before your appointment, please bring your policy number and ID card. Have you confirmed your acupuncture coverage with your insurer? Coverage amounts, annual limits, and referral requirements vary by plan, so it is important to verify your benefits in advance.
If your provider is not listed, a detailed receipt will be provided for reimbursement so you can submit the claim directly to your insurer.
